Continuous-Care Program

Continuum Plenya

Health is not solved in one consultation. It is built over time — together, with method. Six or twelve months led by the Plenya Team, guided by the Plenya Score, organized by The ACTS Method.

The Problem

The conventional check-up does not look for what will take you out of the game. By the time it finds it, you are already sick.

Between the lab that reads "normal" and the diagnosis that changes your life lies a silent window of ten to twenty years. Inside it, small symptoms accumulate, biomarkers begin to drift within the "acceptable range" — and no one investigates, because no one is looking.

Reacting only after problems show themselves is the most expensive bet you can place on your own health. The Continuum exists for those who decided to step out of that pattern.

About the Program

Some look for health once something has shown up. Continuum Plenya exists for those who chose the opposite path.

A Plenya Consultation begins your relationship with your managing physician. The Continuum deepens that relationship over time — months or years with the clinician who oversees your care longitudinally and aligns your plan between Plenya's team and the doctors already caring for you outside the clinic (cardiologist, endocrinologist, gynecologist). Twenty years of clinical practice taught a simple truth: living longer is not living well. Health is built over time — together, with method, and with presence — or it is not built at all.

It begins with the complete Plenya Score — a full clinical, laboratory, behavioral and functional assessment applied by the team — and continues in steady rhythm: a weekly meeting with one of the four professionals, a cycle that completes every four weeks, quarterly re-evaluations, and a structured close at the end of the period. No pillar of The ACTS Method goes unreviewed.

Recognition

Continuum Plenya is not for everyone.

It is for those who recognize one of these moments. Not as a diagnosis — but as the point in life when it makes sense to build health over time, together, with method.

Doing everything right — and the signals don't add up.

Trains, eats well, sleeps. Even so, energy doesn't return, the gut acts up, the skin loses its glow, three viruses a year. Small symptoms that, together, describe a terrain wearing down.

The wake-up call landed.

A lab result that startled you, a family diagnosis, a birthday that hurt. The window seemed far away and suddenly came too close.

Carrying more than yourself.

Partners, team, children, parents — people depend on you whole. Health became the foundation of what you sustain.

In a transition no one names.

Sleep, mood, body — something is changing and the clinical answer is "it's nothing yet" or "it's just age." There's no map.

Too much information, not enough integration.

Trainer, dietitian, therapist, physician — each in their own corner. Wearables, dashboards, reports — data in surplus, decisions missing.

Wanting to be whole for those still to arrive.

A small child at 45, projects that don't fit in ten years. The question is no longer how long will I live — it became how will I be when they need me.

Honesty

Who the Continuum is not for.

The earlier we align expectations, the better the care works — and the more we respect the time of those still to arrive.

  • Anyone seeking a one-off solution.

An acute symptom, a specific question, one set of labs to interpret — that's solved in a Plenya Consultation. The Continuum is a six- or twelve-month commitment.

  • Anyone unwilling to change anything.

The Continuum will not change what you don't want to change. But it also will not work without willingness. The team proposes, returns the data, adjusts together — you execute.

  • Anyone who needs urgent care.

We are not an urgent-care service. The WhatsApp group answers questions during business hours — it does not replace an emergency room or on-call clinic.

In Doubt?

Not sure if any of those portraits is yours?

Five short questions return which portraits of the program most match your moment. No diagnosis, no data capture.

Team

Four specialists. One single plan.

Physician, nutritionist, psychologist, and exercise physiologist meet before your first encounter, discuss your case, and design an integrated plan. Every clinical decision speaks to the others.

How It Happens

One front per week. All present every month.

In the four-week cycle, each pillar of The ACTS Method is addressed by a responsible professional — under the longitudinal clinical management of your managing physician, who maintains continuity of the plan and coordinates with the doctors caring for you outside Plenya. No pillar goes without monthly review.

The Timeline

From day zero to the next horizon.

01

Week 1

Four online consultations.

An online consultation with each of the four professionals at different points in the week — starting with the physician. Together, they fill out the Plenya Score, which sets the starting point — and the direction. When indicated, additional labs are ordered.

02

Week 2

Team meeting and plan opening.

The four professionals meet, discuss the case, and build an integrated initial plan aligned with your goals. Then, in an online opening session, the physician returns the clinical reading and presents the full plan. No one prescribes alone.

03

Following weeks

Weekly meeting, on rotation.

Each week, an online meeting with one of the four professionals. Every four weeks the cycle completes — every area of ACTS is reviewed and reinforced month after month.

04

As the plan evolves

The Plenya Box.

After the plan is built, a personalized box arrives at your door — curated items from the team and supplements or compounded medications specific to your protocol. As care evolves and the plan adjusts, new boxes follow the same rhythm — prepared whenever clinically indicated.

05

Every three months

Score re-evaluation.

Follow-up labs as clinically indicated and a new round of the Plenya Score. The curve shows what advanced, what plateaued, and where the next intervention enters.

06

End of cycle

Final assessment and next horizon.

At the end of six or twelve months, a structured assessment closes the period. From there, we decide together how to design the next cycle.

Perguntas frequentes

Frequently asked questions about the Continuum.

  • Every Plenya Consultation already begins the relationship with the managing physician. The Continuum deepens that relationship over time — multidisciplinary team discussing your case, a single written plan, weekly meetings, and quarterly re-evaluations. The Plenya Score shows the evolution as a curve.
  • Yes. The Continuum is complementary. Clinical coordination stays with the Plenya physician, but ongoing treatments with other professionals continue — with integrated reading, and nothing is changed without alignment.
  • The first clinical reading happens at the initial consultation itself. The Score curve typically shows movement between the first and third months — that's when the first adjustments mature.
  • 100% online. All weekly team meetings happen via video call. The Plenya Box ships to you by mail.
  • Curated items chosen by the team and supplements or compounded medications specific to your protocol, based on clinical indication. As care evolves, new boxes are prepared.
  • Through direct conversation with the team, after an initial alignment call. Conditions and payment options are presented based on the modality you choose (six-month or annual).
  • Yes. Cancellation conditions are presented before enrollment and follow Brazilian consumer protection law.
