Method

ACTS — four pillars for health that sustains over time.

Plenya's clinical framework organizes care into four interdependent dimensions of health. It is not a protocol. It is the way we decide, monitor, and follow up — with the depth that longevity demands.

Premise

The body does not wait for the diagnosis to fall ill.

By the time blood sugar climbs, insulin resistance has been building for a decade. By the time a heart attack happens, the plaque has been growing for twenty years. The window for intervention exists — but it closes in silence.

The ACTS Method is how we operate inside that window. Four interdependent pillars that, together, organize preventive care in a structured way — labs, data, clinical decision-making — directed at what comes before disease.

Pillar 01

A

Activity, Alimentation & Smart Adjuncts

The three engines no medication can replace

Food is not fuel — it is information. Every meal instructs genes, modulates hormones, and feeds the microbiome. Exercise is one of the most powerful medications that exists. Supplementation corrects what the lab shows is missing. The three engines work together.

Medication takes care of what it reaches. The rest — insulin, inflammation, sarcopenia, microbiome — depends on what you do every day.

7 clinical pillars

Nutrition (Quality & Pattern)Food Tolerance & SupplementationPhysical Activity (Habit)Physical Capacity (Testing)Adiposity & Cardiometabolic RiskMuscle Mass & Cellular HydrationTrajectory & Origins

What we monitor · examples

  • Body composition and VO₂ max
  • Eating pattern and feeding window
  • Strength, flexibility, and aerobic capacity
  • Vitamin D, B12, iron, and omega-3 status

Partial list — actual monitoring covers dozens of additional markers per pillar, tailored to your case.

Pillar 02

C

Clinical Optimization

The expanded reading no conventional checkup delivers. Integrated care, adjusted each cycle.

Hormones, biomarkers, data-guided pharmacology. The part of the work where the physician stops being a lifestyle counselor and becomes the pilot of a data dashboard — test, intervene, retest, adjust.

Ordering TSH alone is like checking the faucet pressure without verifying that water is coming out. Metabolic management begins by ordering the right test.

What we monitor · examples

  • Full hormone panel (free testosterone, SHBG, estradiol, DHEA-S, cortisol, thyroid)
  • ApoB, lipoprotein(a), CAC when indicated, measured blood pressure
  • Cystatin C, albuminuria, eGFR; ALT, AST, GGT, hepatic steatosis on imaging
  • Insulin, HOMA-IR, HbA1c, C-peptide
  • VO₂ max, grip strength, segmental body composition
  • High-sensitivity CRP, homocysteine, ferritin, omega-3 index, vitamin D, B12
  • Cognitive screening, gut microbiota and age- and profile-targeted cancer screening; genetic markers when indicated

Partial list — actual monitoring covers dozens of additional markers per pillar, tailored to your case.

Pillar 03

T

Tending Mind, Body & Bonds

Memory, energy, mood, stress and bonds: emotional symptoms and biology treated in the same plan.

The axis between psychology, the immune system, and inflammation. Psychological states directly alter immune, hormonal, and inflammatory function — not as metaphor, as mechanism. When the HPA axis is on chronic alert, no other pillar functions fully.

Ana optimized everything biochemistry would allow. The hs-CRP would not come down. What needed treatment was the anxiety she called personality.

What we monitor · examples

  • Morning cortisol and daily pattern
  • hs-CRP and inflammatory markers
  • Heart rate variability (HRV)
  • Anxiety, depression, and burnout screening
  • Quality of relationships and social support

Partial list — actual monitoring covers dozens of additional markers per pillar, tailored to your case.

Pillar 04

S

Sleep, Rhythm & Recovery

Sleep, rest, routine, schedule and light: the base of the other three pillars.

Sleep, chronobiology, the synchronization of biological clocks. The suprachiasmatic nucleus is the conductor; every cell has its clock. When signals arrive at unpredictable times, the orchestra goes out of tune — and the name for that is chronodisruption.

When the substrate is compromised, everything built above starts to crack. Sleep is not a pillar like the others — it is the base.

What we monitor · examples

  • Sleep duration, efficiency, and architecture
  • Symptoms and screening for obstructive sleep apnea
  • Chronotype and morning light exposure
  • Feeding window and meal timing

Convictions

Five sentences that took a decade to become convictions.

01

The body does not wait for the diagnosis to fall ill.

Disease begins ten, fifteen, twenty years before the label. The window for intervention exists — but it closes. And it closes in silence.

02

No one changes everything at once.

Two focuses per quarter, with measure and deadline. Those who try to change ten things on Monday usually have changed none by Friday.

03

The pillars do not work in isolation.

Nutrition without sleep is wear-down. Exercise without recovery is overtraining. Hormones without mental health is optimization on sand.

04

The partnership matters.

No patient does it alone. Preventive medicine is networked work — physician, multidisciplinary team, and the patient at the center.

05

The best time to start was ten years ago. The second-best time is today.

This is not a motivational line. It is clinical data. Reversibility decreases over time. The cost of inaction increases with age.

How We Apply It

Applied by a team. Measured by the Plenya Score.

The ACTS Method is the framework that organizes our clinical decisions and our communication with you. The Plenya Score translates your current state into a number that evolves throughout follow-up — so progress is clear, measurable, and shared.

The rule of two operates here: we begin with two focuses, with clinical judgment, and advance at the pace of your body — not at the pace of well-meaning lists.
